HIV Care & Case Management

HIV and AIDS can be managed successfully with our multidisciplinary approach.

HIV/AIDS services at CDCHC include:

  • Primary care services provided by our physicians, nurse practitioners, and physician assistants
  • Medical case management provided by a nurse with extensive knowledge and experience with HIV care
  • Social services provided by two case managers who assist with referrals for food, housing, DSHS or other financial assistance programs
  • Adherence counseling provided by the entire team of medical providers, nurse, and social service case managers

The Health Resources and Services Administration (HRSA) of the U.S. Department of Health and Human Services (HHS) provides federal funding for HIV/AIDS programs to community based organizations to support outpatient ambulatory health service and support services through Early Intervention Service (EIS) program grants. Country Doctor Community Health Centers receives this federal funding through The Ryan White HIV/AIDS Care Act.
